In the event of emergency, We do hereby properly appoint guardianship of YOUR
CHILDREN'S NAME BORN
to THE PERSON'S NAME and to THE PERSON'S NAME to live with them at STREET ADDRESS until such a time she maybe returned to her natural parent.
THE PERSON'S NAME and to THE PERSON'S NAME have full legal custody of YOUR CHILDREN'S NAME. This means that THE PERSON'S NAME and THE PERSON'S NAME, have the right to have CHILDS' NAME treated by Medical Professionals, Take her to and from schools and all the rights of legal guardians.
